Jane brings over 25 years of experience in Family Medicine to the practice.
She has spent most of her career practicing in medically underserved settings where she enjoyed building relationships with families and their communities. She has had the opportunity to create and manage two outpatient clinics and has served as medical director of a community health center and a nursing facility.
Previously, Jane served as chief medical officer and sole provider for 2,000 patients at the Rockbridge Area Health Center. She also was the founder and co-owner of Tidewater Family Medicine, where she created and managed the outpatient primary care office. Additionally, Jane was co-owner and administrator for Maury River Family Practice, which brought care to medically underserved areas of Rockbridge County, Virginia.
Jane holds an M.D. from The Jefferson Medical College of Thomas Jefferson University.
Dr. Janki Rami, DO, MPH, serves as Medical Director of Primary Care at Centivo. She is a Board-Certified Family Physician and brings over 10 years of clinical experience in family medicine with a focus on women's health. As a physician leader, she leads clinical teams, combining clinical expertise with strategic vision to drive healthcare innovation and improve patient outcomes. She emphasizes preventive care and personalized health strategies, ensuring quality and compassion in every interaction. With her clinical and health services management experience, Dr. Rami shapes forward-thinking approaches to primary care, delivering accessible and modern healthcare to diverse communities.
Dr. Raj Shah is the Senior Medical Director at Centivo Care and a Board-Certified Internal Medicine Physician. He earned his medical degree from Chicago Medical School and completed his residency training at the University of Michigan. With a strong background in biomedical engineering from his undergraduate studies, he combines a data-driven mindset with a human-centered approach to care.
Before joining Centivo Care, Dr. Shah served as a primary care physician and leader at Northwestern Medicine, one of Chicago’s leading health systems. Throughout his career, he has remained deeply passionate about primary care, believing that trust and compassionate care are the foundations of every meaningful patient relationship.
In addition to building strong connections with patients and helping them achieve their unique health goals, Dr. Shah plays an active role in advancing healthcare delivery at Centivo Care. He is committed to developing innovative, sustainable, and thoughtful approaches to making care more effective for all.
Sydney Holland is a Physician Assistant in Centivo Care's Practice.
Sydney was born and raised in eastern Wyoming/western Nebraska. She graduated from Wichita State University with her Master of Physician Assistant degree in 2016. From there she worked at a family practice clinic in Oklahoma. Recently, she and her husband moved back to Wichita to be close to his family.
Sydney enjoys the holistic aspect of family medicine. She enjoys palliative care, management of acute and chronic conditions, and building relationships with her patients and their families.
Her interests include traveling, going to the lake, spending time with friends and family, playing sports and other outdoor activities, and spending time with her pets.
Gaytri Kumar is a board certified Physician Assistant who graduated from St. John's University in New York in 2012. She specializes in Internal Medicine with additional experience in Nephrology as well as Quality and Patient Safety. Gaytri wants every patient to feel supported and confident that their healthcare needs will be met. She prides herself in taking a holistic approach with her patients in effort to motivate positive change. She believes Eden Health's forward-looking mission will provide this to their patient's and set a new standard of care for the future.
I am a board-certified Family Nurse Practitioner with an MSN from UCLA, specializing in lifestyle and functional medicine. With over 15 years of experience in healthcare—including five years in primary care—I have also worked as a Functional Medicine Practitioner and Integrative Nutrition Health Coach. My passion lies in addressing the root causes of health issues and empowering individuals to achieve lasting wellness through personalized, evidence-based care.
Currently, I am pursuing a Doctorate in Nursing Practice (DNP) at Florida State University, focusing on lifestyle medicine and psychiatric mental health. My goal is to bridge the connection between physical and mental well-being.
Outside of work, I enjoy surfing, practicing yoga, exploring nature, and fostering meaningful connections with my family and community.
Hi, my name is Kim. I am a board-certified Family Nurse Practitioner. Prior to nursing, I earned a BS in neurology, biology, and physiology from UC Davis. I received my BSN from the University of Rochester and my MSN from Columbia University. I have worked in primary care, urgent care, retail health, and neurology. Hi! I'm Brittany. I am a Board Certified Physician Assistant. I received my Masters of Medical Science from St. Francis University. I've practiced Emergency Medicine, Urgent Care and Telemedicine and taught in the University of MD PA program. I am Baltimore born and raised. I am passionate about providing equitable care, meeting patients where they are and helping patients become advocates for their health. In my spare time, I enjoy shopping for vintage clothes, hiking and wandering around new places, traveling and catching live music whenever I can. I'm Lindsey Heath, one of the Physician Assistants here at Eden. I went to PA school at The University of North Texas Health Science Center in Fort Worth, TX and graduated in 2014. Prior to this, I earned a Master's degree in Community Public Health at New York University in 2009. I have been working as a Primary Care Physician Assistant for the past 10 years. My prior experience in community public health has helped shape my values and approach to practicing medicine. I am dedicated to being an advocate for my patient's healthcare needs. I pride myself on delivering a personalized, high quality healthcare experience. My name is Lee Frerker and I have the privilege of being one of the many talented providers joining Centivo Care. I received my bachelor’s degree from the University of Illinois at Champaign-Urbana and my master’s degree from Midwestern University. I have been working in medicine as a board-certified PA for over fourteen years with the majority of my time in primary and urgent care. I have been focusing on virtual care since the pandemic started and am amazed with the excellent clinical outcomes and patient satisfaction this type of medicine brings.

I'm Michelle Guillot, a Physician Assistant, specializing in Internal Medicine at Eden Health. Originally hailing from Louisiana, I began my journey in healthcare by earning a Bachelor's degree in Biology from Louisiana State University in Baton Rouge. Continuing my education, I pursued my passion at LSU Shreveport, where I completed my Physician Assistant degree. My approach to patient care is deeply rooted in compassion and personalization. I believe in tailoring treatment plans to meet each patient's individual needs, ensuring they achieve the highest degree of happiness and health possible. Empowering patients to take control of their health and granting them autonomy in their healthcare decisions is paramount to me. Listening attentively to my patients is at the core of my practice. I shy away from quick, one-size-fits-all diagnoses, preferring instead to craft comprehensive and tailored care plans that align with each patient's lifestyle and aspirations. Outside of medicine, I'm an adventurer at heart. I also have a soft spot for comedy shows, as laughter is truly the best medicine.
