What we do

Our services include, but are not limited to:

Primary care with integrated behavioral health
Preventive care (like annual health screenings, vaccination guidance and referrals)
Referrals to specialists
Minor illnesses and urgent care (such as colds, flu, COVID-19, aches and pains), with after hours access
Prescriptions and refills
Chronic care (for conditions like diabetes, high blood pressure and heart disease)
X-ray and lab orders

Our office hours are Monday–Friday, 9 am–8 pm ET

Non-business hours
As a patient of Centivo Care, you have access to a clinician after-hours. During non-business hours, an on-call provider is available for urgent matters. You can reach out to your team conveniently through the Centivo Care app.
Sending medical records
If you or your doctor need to send us your medical records, reach out to us through the Centivo Care app for fax instructions.

If you have a life-threatening emergency, call 911 or go to the nearest emergency room.
