Jane brings over 25 years of experience in Family Medicine to the practice.

She has spent most of her career practicing in medically underserved settings where she enjoyed building relationships with families and their communities. She has had the opportunity to create and manage two outpatient clinics and has served as medical director of a community health center and a nursing facility.

Previously, Jane served as chief medical officer and sole provider for 2,000 patients at the Rockbridge Area Health Center. She also was the founder and co-owner of Tidewater Family Medicine, where she created and managed the outpatient primary care office. Additionally, Jane was co-owner and administrator for Maury River Family Practice, which brought care to medically underserved areas of Rockbridge County, Virginia.

Jane holds an M.D. from The Jefferson Medical College of Thomas Jefferson University.

Dr. Janki Rami Medical Director - Primary Care

Dr. Janki Rami, DO, MPH, serves as Medical Director of Primary Care at Centivo. She is a Board-Certified Family Physician and brings over 10 years of clinical experience in family medicine with a focus on women's health. As a physician leader, she leads clinical teams, combining clinical expertise with strategic vision to drive healthcare innovation and improve patient outcomes. She emphasizes preventive care and personalized health strategies, ensuring quality and compassion in every interaction. With her clinical and health services management experience, Dr. Rami shapes forward-thinking approaches to primary care, delivering accessible and modern healthcare to diverse communities.

Dr. Raj Shah Senior Medical Director - Care Delivery

Hello, I am Dr. Shah. After completing my undergraduate degree in biomedical engineering, I started at Chicago Medical School where my passion for primary care was born. After training, I decided to experience travel medicine with my wife and worked at Guam Memorial Hospital which serves the indigenous community of Guam and its neighboring Pacific islands. There, my wife and I became advanced certified in scuba diving and fell in love with the life and vastness of the ocean. After moving back to Chicago, I practiced primary care medicine and worked on improving healthcare delivery at Northwestern Medical Group. I enjoy forging strong relationships with patients and helping them achieve their specific health goals. For me, the foundation of any patient relationship is trust and making each individual patient feel that they are genuinely cared for. In my free time, I love playing basketball and know how to juggle (though not at the same time!).

Cristina Sumilang Senior Team Lead/Nurse Practitioner

Hi, I’m Cristina! I am a Board Certified Family Nurse Practitioner. I received my Bachelor’s of Science in Nursing from the University of South Florida and worked as a Registered Nurse at a NYC hospital for 8 years prior to completing my Master’s Degree in Nursing at Columbia University. I then continued my education at Columbia to pursue my Doctor of Nursing Practice. As healthcare is evolving, I am passionate about creating innovative healthcare strategies that are of the highest quality and efficiency, but also economically feasible. I offer the most up-to-date, evidenced-based treatment while practicing with a holistic approach to care. I strive to create an environment where patients feel empowered to optimize their physical, emotional, and spiritual well-being. I believe that the relationship between a medical provider and patient is a collaborative one, in which they work together as partners to achieve health and lifestyle goals.

Sydney Holland Physician Associate

Sydney Holland is a Physician Assistant in Centivo Care's Practice.

Sydney was born and raised in eastern Wyoming/western Nebraska. She graduated from Wichita State University with her Master of Physician Assistant degree in 2016. From there she worked at a family practice clinic in Oklahoma. Recently, she and her husband moved back to Wichita to be close to his family.

Sydney enjoys the holistic aspect of family medicine. She enjoys palliative care, management of acute and chronic conditions, and building relationships with her patients and their families.

Her interests include traveling, going to the lake, spending time with friends and family, playing sports and other outdoor activities, and spending time with her pets.

Gaytri Kumar Quality Manager/Physician Associate

Gaytri Kumar is a board certified Physician Assistant who graduated from St. John's University in New York in 2012. She specializes in Internal Medicine with additional experience in Nephrology as well as Quality and Patient Safety. Gaytri wants every patient to feel supported and confident that their healthcare needs will be met. She prides herself in taking a holistic approach with her patients in effort to motivate positive change. She believes Eden Health's forward-looking mission will provide this to their patient's and set a new standard of care for the future.

Kim Quach Nurse Practitioner

Hi, my name is Kim. I am a board-certified Family Nurse Practitioner. Prior to nursing, I earned a BS in neurology, biology, and physiology from UC Davis. I received my BSN from the University of Rochester and my MSN from Columbia University. I have worked in primary care, urgent care, retail health, and neurology. I practice evidence-based medicine and work with patients to individualize lifestyle goals. I look forward to meeting you. One fun fact about me: I have two cats!

Brittany Thomas Physician Associate

Hi! I'm Brittany. I am a Board Certified Physician Assistant. I received my Masters of Medical Science from St. Francis University. I've practiced Emergency Medicine, Urgent Care and Telemedicine and taught in the University of MD PA program. I am Baltimore born and raised. I am passionate about providing equitable care, meeting patients where they are and helping patients become advocates for their health. In my spare time, I enjoy shopping for vintage clothes, hiking and wandering around new places, traveling and catching live music whenever I can. I am also a certified yoga instructor and a lover of movement. I look forward to working with you to achieve your healthcare goals.

Lindsey Heath Physician Associate

I'm Lindsey Heath, one of the Physician Assistants here at Eden. I went to PA school at The University of North Texas Health Science Center in Fort Worth, TX and graduated in 2014. Prior to this, I earned a Master's degree in Community Public Health at New York University in 2009. I have been working as a Primary Care Physician Assistant for the past 10 years. My prior experience in community public health has helped shape my values and approach to practicing medicine. I am dedicated to being an advocate for my patient's healthcare needs. I pride myself on delivering a personalized, high quality healthcare experience. I have been working in virtual primary care for the past 2 years and am happy to be working for an organization that is innovating the delivery of health care.

Brian Locsin Nurse Practitioner

Hello! I am Dr. Locsin, a board-certified adult nurse practitioner. I earned my Master of Science in Nursing and Doctor of Nursing Practice from the City University of New York, Hunter College. My experiences as a nurse practitioner include primary care, rheumatology, men's health, and obesity medicine. I don't believe in a one-size-fits-all mindset in medicine, so you will find that I tailor treatment based on strong scientific evidence and based on each individual's physiologic characteristics and preferences. My name is Lee Frerker and I have the privilege of being one of the many talented providers joining Centivo Care. I received my bachelor’s degree from the University of Illinois at Champaign-Urbana and my master’s degree from Midwestern University. I have been working in medicine as a board-certified PA for over fourteen years with the majority of my time in primary and urgent care. I have been focusing on virtual care since the pandemic started and am amazed with the excellent clinical outcomes and patient satisfaction this type of medicine brings.

Mariella Castro Physician Associate

Hi, I'm Mariella! I am a Board Certified Physician Assistant. I received a Bachelor’s Degree in Political Science at UCLA. Soon after, I made my journey to NYC, where I received a Master's of Science in Physician Assistant Studies, and was inducted into the Pi Alpha National Honor Society. I have worked in primary care for several years, and have enjoyed treating culturally and socially diverse populations. I am passionate about caring for patients in a holistic manner. This means recognizing mind, body, and spirit as equal contributors to a person's overall health. I'm Michelle Guillot, a Physician Assistant, specializing in Internal Medicine at Eden Health. Originally hailing from Louisiana, I began my journey in healthcare by earning a Bachelor's degree in Biology from Louisiana State University in Baton Rouge. Continuing my education, I pursued my passion at LSU Shreveport, where I completed my Physician Assistant degree. My approach to patient care is deeply rooted in compassion and personalization. I believe in tailoring treatment plans to meet each patient's individual needs, ensuring they achieve the highest degree of happiness and health possible. Empowering patients to take control of their health and granting them autonomy in their healthcare decisions is paramount to me. Listening attentively to my patients is at the core of my practice. I shy away from quick, one-size-fits-all diagnoses, preferring instead to craft comprehensive and tailored care plans that align with each patient's lifestyle and aspirations. Outside of medicine, I'm an adventurer at heart. Whether I'm backpacking through scenic landscapes, soaring through the skies while paragliding, or scaling rock faces, I find solace and excitement in exploring the world around me.
