How to activate with Centivo Care

If you're on a Centivo health plan that requires you to activate by choosing a primary care doctor, it’s easy once you create your Centivo online account. Just follow the step-by-step process.

Step 1

Create your Centivo account by downloading the Centivo app from your favorite app store or text “DOWNLOAD” to 65021.

Or go to  my.centivo.com. Click First time? Create an account and follow the prompts.

Step 2

The first time you log in to your account, you’ll be asked to designate a primary care doctor for yourself and any family members on your plan. Click Add a doctor.

Step 3

Click Enter my preferences to find a primary care doctor.

Step 4

Make sure the box next to Include virtual doctors is checked, then select Next.

Step 5

Click on Centivo Care.

Step 6

Click Choose this doctor.

Step 7

If you have eligible dependents, you can choose the same provider for them too.
