Learn more about Centivo Care and how to get started.

  • Centivo Care is a convenient, virtual primary care option that takes the place of in-person care. You can visit with your doctor online from the comfort of your home, at work or on the go. Using the secure and easy-to-use app, you can schedule an appointment and communicate with your doctor, refill your prescriptions, view your medical chart and more. Plus, as a patient of Centivo Care, you have after-hours access to a clinician for urgent needs. If you need to be seen in person, your dedicated team will help arrange in-person care with a local, quality in-network provider.

  • Your care team includes licensed mental health providers. If your team thinks you’ll benefit from working with a mental health provider, they’ll arrange for you to meet with them. If further treatment is required, they can refer you to an in-network behavioral health specialist.

  • If you need to be seen for an urgent matter that doesn’t require you to go to the emergency room, same day appointments are available for established patients and we also have after-hours support. Some common issues that you may need to see someone quickly for include sinus infections, cold, flu, COVID-19 or rashes.

  • When you choose Centivo Care, this replaces your current primary care doctor. Your dedicated Centivo Care team will get to know you and will be your first stop for all your care. If your current doctor isn’t in the Centivo Network, Centivo Care is a convenient option to consider.

  • While most care can be provided by your doctor on a phone or video call, there may be times when you need to be seen in person. If this happens, your Centivo Care team will guide you to in-person care with a quality, in-network provider near you. Your Centivo Care team can also order labs and imaging at a local facility and the results will be sent back to your team.

  • All visits with your Centivo Care Team are FREE.* If you need care outside of your team, such as seeing a specialist, getting blood work or an MRI, you may have out-of-pocket costs based on your plan details.

  • Centivo Care is a convenient alternative to in-person care. You can get all your primary care at home, from work or on the go, whenever you need it – for FREE!* You can communicate with your care team by phone or through the secure and easy-to-use app. You can schedule appointments and communicate with your doctor, refill your prescriptions, view your medical chart and more. It’s a great choice if you don’t have a primary care doctor already. And if you need to be seen for anything urgent or after hours, you have access to care. If you decide you prefer an in-person primary care doctor, you can switch at any time throughout your plan year.

  • The Centivo Care team is available Monday–Friday, 9 am–8 pm ET through the secure Centivo Care app. An on-call provider is available for urgent matters after-hours. You can also use the app to make appointments, refill a prescription and view your medical chart. If you have a life-threatening emergency, call 911 or go to the nearest emergency room.

  • Yes, if you’re traveling, you can still contact your Centivo Care team for any care needs and they will help coordinate your care needs as appropriate, based on your location.

  • Centivo Care is a convenient option offered with many Centivo plans. You can become a patient if:

  • The Centivo Care team can provide care in Spanish as well as 150 other languages.

  • The Centivo Care app is the primary way you’ll communicate with your team. It’s available on your mobile device. The app makes it easy to make appointments and talk with your care team, refill prescriptions and view your medical chart.

  • Absolutely! It’s important to find a care team that’s the right fit for you. If you’re enrolled in a Centivo health plan, we make it easy to change your designated primary care doctor in the Centivo app or portal, or by calling Centivo Customer Support.

  • For members enrolled in a Centivo health plan, there are 3 easy ways to activate with Centivo Care:

    • On the Centivo app: Text “DOWNLOAD” to 65021 or visit your app store
    • Online: Go to my.centivo.com
    • By phone: Call Centivo Customer Support using the phone number on the back of your ID card.

  • Prior to becoming a patient, you can reach out to us by clicking “Get In Touch” at the top of this page. Once you become a Centivo Care patient, you can reach out via the Centivo Care app.
